FOURSOME ENTRY

(1) Foursome entry is good for (2) 2-player team admissions to the UGP Invitational at The Riviera Country Club on August 11, 2025. Entry includes:

• 18 holes of golf for four players

• Format: 2-Player Scramble

• Treatment and warms-ups provided by the UGP Recovery Team

• Pre-round breakfast, on-course food & beverage, and post-round reception dinner

• Open bar all day

• Live music

• Arrival gifts provided by UGP and our partners

• Forecaddie for each 4-person group

• Premium prizes and awards

• Discounted golf club shipping through ShipSticks

Please Note: Foursome entries must specify all players’ names and contact info for each team upon checkout. Teams will be paired based on the order in which player names are submitted. If any player is unconfirmed, please enter “TBD” when completing the form.

All net proceeds will go towards supporting the LAFD Foundation and their mission to provide essential equipment, resources, and programs to assist the Los Angeles Fire Department in protecting our beloved community.

CANCELLATION POLICY

We understand that plans can change, and we’ll always do our best to accommodate when possible. That said, due to the nature of tournament planning and limited capacity, refund requests will only be considered under the following guidelines.

Please note: For all cancellations, UGP is not responsible for finding a replacement if you, your partner, or your team are unable to attend. Single-player cancellations within a twosome or foursome are responsible for finding their own replacement and are not guaranteed a refund.

For cancellations made…

• More than 60 Days Out: A full refund will be provided upon request.

• 30-60 Days Out: A full refund will be provided only if a replacement team or individual player is found. In the event a replacement is not found and a refund cannot be provided, a partial credit may be issued toward a future UGP Tournament, subject to eligibility on a case-by-case basis.

• Less than 30 Days Out: Refunds are not available for cancellations made less than 30 days of the tournament. Players may transfer their individual/team spot to a replacement player/team or forfeit that individual’s/team’s entry fee.
